Key Responsibilities

  • Manage real-time caregiver-to-client scheduling, ensuring optimal alignment of skill sets, geographic proximity, and availability to maximize the quality and continuity of care.
  • Oversee incoming care requests and coordinate timely responses within established service level agreements (SLAs), maintaining client satisfaction.
  • Maintain and update scheduling software systems such as ClearCare, AlayaCare, and AxisCare with accurate client and caregiver details for effective record keeping and operational transparency.
  • Serve as a vital communication liaison, proactively confirming schedules, addressing any changes, and promptly managing coverage gaps with caregivers and clients.
  • Collaborate closely with care managers and back-office teams to ensure accurate incident reporting and follow up on staff-related issues.
  • Identify and resolve scheduling inconsistencies such as caregiver no-shows or last-minute cancellations by dynamically adjusting the scheduling matrix.
  • Analyze staffing coverage and generate productivity reports to support operational decision-making and continuous improvement.
  • Support additional operational workflows including timesheet collection, payroll preparation, and general administrative follow-up when capacity allows.
  • During the first week: complete system training; shadow experienced schedulers and care managers; begin drafting schedules under supervision; familiarize yourself with caregiver/client preferences and recurring care needs.

Requirements

We seek a highly organized and detail-oriented candidate with at least 5 years of experience in home health or healthcare scheduling, patient coordination, or similar roles. The ideal candidate is proficient with scheduling platforms such as ClearCare, AlayaCare, or AxisCare and demonstrates strong command of English with a neutral accent suitable for effective communication with diverse stakeholders.
Key traits include excellent organizational skills, time management under shifting priorities, and the ability to maintain composure and efficiency in a fast-paced environment. A strong understanding of HIPAA compliance and familiarity with U.S. healthcare data protocols is essential to protect sensitive information throughout all interactions.
The candidate must be comfortable working during Pacific Standard Time (PST) business hours, coordinating with distributed teams and clients.

Desirable Skills and Experience

While not required, experience in revenue cycle management, timesheet or payroll processing would be advantageous to broaden operational support capabilities. Prior exposure to home care agency operations will facilitate quicker integration into our workflow.
Familiarity with VoIP communication systems such as OpenPhone, Klara, or RingCentral and patient intake platforms will be beneficial for enhanced client and caregiver communications.
